DHS Privacy Release Form

If you or a loved one has an open order of removal or are worried about getting picked up by ICE, consider filling out this DHS Privacy Release form in advance so our office can intervene.

Congressional offices need this form to make inquiries about your location, transfers, medical attention, and circumstances around your arrest. 

You can keep a copy with a loved one or lawyer, and they can contact your congressional representative in the event that you are detained.

Please note that it is only valid for 90 days from the date of signature.
